No, Medicare Part D is not entirely free. You may qualify for the Extra Help financial assistance program if your income is below a certain threshold. Some Part D plans may be premium-free, but you will still have co-payments for your medications at your plan’s preferred pharmacies. 

Are all prescription drugs covered under these plans?

Each plan is required to meet government standards so that there are medications for every health issue. So while it will offer medicines used to control diabetes, it may not provide every medication. Do I need to get a prescription drug plan if I don’t use medications regularly?

You are not required to join a PDP, but it is an excellent decision to join when you reach your Initial Enrollment Period. When you join later, you will be assessed a fee of 1 percent of the national base premium for each month that you did not have coverage. For 2021 that national base premium is $33.06. 

So if you delay coverage for four whole years, going forward, you now pay 48 percent of $33.06 every month. The amount is rounded to the nearest $0.10, so the fee would be $15.90 added to your monthly premium. It may be better to choose a low-cost plan that offers basic coverage at first. Then, you can switch to a more suitable plan when your health needs change.
